About Patient-Centered Medical Home
Bluesky Health is a patient-centered medical home. It is built on the patient care principles of a personal medical home called for by the American Academy of Family Physicians (AAFP) in its 2004 groundbreaking Future of Family Medicine (FFM) report. This concept has now gained the attention of other national primary care organizations, and they have joined with the AAFP in calling for a patient-centered medical home as the foundation for primary care in this country.
Additionally, Bluesky Health endorses the 6 goals principles laid out by the Insitute of Medicine (IOM) in its 2001 report Crossing the Quality Chasm: A New Health System for the 21st Century. It said that the 21st century health care system should be:
- Safe--avoiding harm to patients from the care initiated to help them
- Effective--delivering evidence-based care to those who will benefit and refraining from using care on those who won’t benefit
- Patient-centered--providing care that is respectful of patient preferences and needs, and guided by their values
- Timely--reducing delays for both patients and providers
- Efficient--avoiding waste of supplies, ideas, energy, and other resources
- Equitable--providing care that is of consistent quality regardless of personal characteristics including ethnicity, geographic location, and socioeconomic status.
These principles heavily influence our system design.
